Officer Info Home & Forms This page has been designed as a resource for current Club Sports and Alpine Club Officers who need to submit a variety of information to the Club Sports Manager and/or the Club Sports Advisory Board throughout the year. Required Paperwork Information (Club Sports & Alpine Club) The following forms should all be completed and submitted online unless it is a form designed to be completed by hand and submitted in person. All participants in Club Sports and Alpine Club must complete an Assumption of Risk Form for each club they participate in, sign/date it, and turn it in to the Club Sports Manager Prior to participating in any club activity. A Supplemental Information Form must be on file with the Club Sports Manager for each participant in Club Sports and Alpine Club. Each member must only submit this form once for Club Sports and once for Alpine Club (if a program participant). If any information on the Supplemental Information Form changes, it it that individual's responsibility to submit a new form to the Club Sports Manager. Any Club Sports or Alpine Club participant that will be using their personal vehicle for club business must completely fill out the section related to insurance and attach a copy of their current, valid Driver's License. *Travel forms must be completed and submitted 7 days prior to the event! This includes Enterprise Rental Request Forms, Travel Request Forms, and all required supplemental materials outlined in the Club Sports and Alpine Club Manuals.
